Dementia Post Diagnostic Support Service - Alzheimer Scotland Dementia Nursing Service (NHS Western Isles)
The Post Diagnostic Support service offers support for people who have been newly diagnosed with dementia.
The service aims to help you self manage symptoms, encourage independence and plan for the future. People usually stay with service for a year and a named worker will manage your case.
We offer:
- education about dementia and particulars of subtype
- information and support to help you manage symptoms
- support to access support services within the community
- introduction to peer support
- monitoring of anti-dementia medications if appropriate
- sign posting carers for support if appropriate
- access to psychological support
- referral to other health or social care supports if appropriate

You should be offered referral to this service by your diagnosing clinician. If this has not been offered referral can be made by your GP, social worker or other health and social care professional.
Clinics can usually be offered at patient homes.
